摘要:
目的研究宫腔粘连(intrauterine adhesions,IUA)患者子宫内膜中赖氨酰氧化酶(lysyl oxidase,LOX)、赖氨酰氧化酶样蛋白-2(lysyl oxidase like 2,LOXL 2)、转化生长因子β1(transforming growth factor-β1,TGF-β1)的表达,并分析上述因子的表达与IUA发生发展的关系。方法选择2016年1月至11月于首都医科大学附属北京妇产医院妇科微创中心收治的中、重度IUA患者20例为研究对象,宫腔镜宫腔粘连分离术中获取IUA周围子宫内膜组织作为研究组(增殖期9例,分泌期11例);选择同期因输卵管性不孕、子宫纵隔的患者20例,宫腔镜术中获取子宫内膜组织作为对照组(增殖期和分泌期各10例)。采用免疫组织化学法检测两组子宫内膜组织中LOX、LOXL 2及TGF-β1的表达情况。结果① 研究组LOX的表达(700±315)低于对照组(1020±242),而LOXL 2、TGF-β1的表达(980±275、880±246)均高于对照组(660±268、500±177)(P<005);② LOXL 2与TGF-β1呈显著正相关(r=0418,P<005);③ 研究组患者临床特征与LOX、LOXL 2及TGF-β1表达的相关性:残留内膜面积<50 %的患者子宫内膜中LOX的表达降低、LOXL 2和TGF-β1的表达升高(P<005);既往流产次数≥3次的患者子宫内膜中LOXL 2的表达升高(P<005);年龄≥32岁、重度IUA患者子宫内膜中TGF-β1的表达升高(P<005)。结论IUA子宫内膜中LOX、LOXL 2及TGF-β1的异常表达与残留内膜面积、AFS评分严重程度、既往流产次数及年龄相关,可能在IUA发生发展过程起到重要作用。
Abstract:
ObjectiveTo investigate the expression of LOX, LOXL 2 and TGF-β1 in endometrium of patients with intrauterine adhesions(IUA) and discuss their relationship with the development of IUA.Methods20 cases with moderate or severe IUA hospitalized in Beijing Obstetrics and Gynecology Hospital from Jan. to Nov. 2016 were selected,in whom undergoing transcervical resection of adhesion were enrolled as IUA group(including 9 cases with proliferative endometrium and 11 cases with secretory endometrium),and 20 cases with hysteroscopy due to tubal infertility and uterus septus as control group(including 10 cases with proliferative endometrium and 10 cases with secretory endometrium).The expression of LOX, LOXL 2 and TGF-β1 in endometrium of the two groups were measured by immunohistochemical method.Results ① The expression of LOX was significantly lower in IUA group (P<005), while LOXL 2 and TGF-β1 were significantly higher in IUA group (All P<005) ② The expression of LOXL 2 was positively correlated with TGF-β1 in two groups(r=0418,P<005) The expression of LOX, LOXL 2 and TGF-β1 correlated with clinical features of IUA:In the condition of residual endometrial area under 50 %, the expression of LOX was lower, while LOXL 2 and TGF-β1 were higher(All P<005);In the condition of more than 3 times abortion , the expression of LOXL 2 was higher(P<005);In the condition of more than 32 years old and severe IUA, the expression of TGF-β1 was higher(P<005).ConclusionThe abnormal expressions of LOX, LOXL 2 and TGF-β1 in endometrium in IUA is possibly correlated with residual endometrial area, abortion times, ages or AFS score It may play a part in the development of IUA.
