摘要:
目的探讨生物反馈电刺激疗法联合盆底肌锻炼在产后盆底功能康复中的应用价值及对患者性功能的影响。方法将2014年10月至2016年7月在钟祥市人民医院接受阴道试分娩的98例产妇用随机数字表法分为观察组和对照组,各49例。对照组进行单纯盆底肌锻炼;观察组在进行单纯盆底肌锻炼基础上联合生物反馈电刺激疗法进行干预。产后随访4个月,对比两组产后1个月及4个月盆底肌张力测试等级分布情况、女性性功能量表(female sexual function index,FSFI)评分、负性情绪[焦虑自评量表(self-rating anxiety scale,SAS)、抑郁自评量表(self-rating depression scale,SDS)]评分,并统计随访期间尿失禁发生率。结果产后1个月,两组盆底肌张力测试等级分布、FSFI、SAS、SDS评分的比较差异均无统计学意义(P>005);产后4个月,观察组盆底肌张力测试等级分布(Z=4193,P=0000)与FSFI评分显著优于对照组(t=5236,P=0000), 观察组的SAS(t=4993,P=0000)和SDS评分显著低于对照组(t=5520,P=0000),两组患者产后压力性尿失禁发生率的比较差异无统计学意义(χ2=2346,P=0126)。结论盆底肌锻炼联合生物反馈电刺激疗法可有效改善产妇产后盆底肌张力及性功能,降低压力性尿失禁发生率,对缓解并消除产妇产后负性情绪具有重要意义。
Abstract:
ObjectiveTo evaluate the application value of biofeedback electrical stimulation combined with pelvic floor muscle exercise in postoperative pelvic floor rehabilitation and its effect on patients' sexual function. MethodsFrom October 2014 to July 2016, 98 pregnant women who received vaginal delivery in Zhongxiang People's Hospital were randomly divided into control group and observation group, with 49 cases in each group. The control group was treated with pelvic floor muscle exercise, and the observation group on this basis was treated with combined biofeedback electrical stimulation therapy. Postpartum follow-up for 4 months, the pelvic floor tension test grade distribution, female sexual function (FSFI) score, negative emotion [anxiety (SAS), depression (SDS)] scores were compared between the two groups at 1 month and 4 months postpartum, and the incidence of urinary incontinence during follow-up were statistically analyzed. ResultsIn the first month after childbirth, There were no significant differences in pelvic floor tension test grade distribution, FSFI scores, SAS scores and SDS scores between the two groups(P>0.05). In the fourth month after childbirth, the pelvic floor tension test grade distribution(Z=4.193,P=0.000) and FSFI scores(t=5.236,P=0.000) of the observation group was significantly superior to the control group, the SAS scores(t=4.993,P=0.000)and SDS scores(t=5520,P=0000) of the observation group was significantly superior to the control group, there were no significant differences about occurrence rate of stress incontinence between the two groups (P>0.05). ConclusionPelvic floor muscle exercise combined with biofeedback stimulation can effectively improve the postpartum pelvic floor muscle tension and sexual function, reduce the incidence of stress urinary incontinence, and it is of great significance to relief and eliminate maternal postpartum negative emotions.
