摘要:
目的探究妊娠期糖尿病(gestational diabetes mellitus,GDM)患者胰岛素抵抗(insulin resistance,IR)与脂联素(adiponectin,ADP)、趋化素(chemerin)和视黄醇结合蛋白4(retinol binding protein-4,RBP 4)的关系。方法选取2016年6月至2017年6月中国科学院大学深圳医院(光明医疗集团)产科收治的120例GDM患者为观察组,同时期行孕检的120例健康孕妇为对照组。对比两组孕妇外周血ADP、Chemerin、RBP 4水平,计算并比较两组胰岛素抵抗指数(homeostasis model assessment-insulin resistance,HOMA-IR)和胰岛β细胞功能指数(homeostasis model assessment-beta cell function,HOMA-β),分析GDM患者血清ADP、Chemerin、RBP 4水平与IR的相关性,通过ROC曲线说明ADP、chemerin、RBP 4以及联合检测在诊断GDM的临床价值。结果① 观察组患者ADP水平和HOMA-IR低于对照组,chemerin、RBP 4水平以及HOMA-β高于对照组,差异均有统计学意义(P<005);② Pearson相关分析显示,空腹血糖水平、空腹胰岛素水平、HOMA-IR与chemerin和RBP 4呈正相关(P<005),而与ADP呈负相关关系(P<005);③ 以灵敏度为纵坐标,1—特异度为横坐标,分别制作ADP、chemerin、RBP 4以及3者联合诊断的ROC曲线,其中3者联合检测在GDM诊断中的灵敏度和特异度最高,分别为865 %和879 %,曲线下面积为0911(0835~0946)。结论GDM患者IR与血清chemerin和RBP 4水平呈正相关,与ADP水平呈负相关;3者联合检测在GDM诊断中具有一定的临床应用价值。
Abstract:
ObjectiveTo explore the relationship between serum adiponectin (ADP), chemerin, retinol binding protein 4 (RBP 4) and insulin resistance(IR) in patients with gestational diabetes mellitus (GDM).Methods120 cases of patients with GDM treated in Shenzhen Hospital, University of Chinese Science Academy from June 2016 to June 2017 were selected as the observation group, and other 120 cases of healthy pregnant women in the same period were selected as the control group. The levels of ADP, chemerin and RBP 4 in two groups of pregnant women were compared.The insulin resistance index (HOMA-IR) and islet beta cell function index (HOMA-β) were calculated and compared between the two groups. To analyze the correlation between serum ADP, chemerin and RBP 4 levels and IR in GDM patients. The clinical value of ADP, chemerin, RBP 4 and combined detection in diagnosing GDM were analyzed by ROC curve.Results① The level of ADP and HOMA-IR in the observation group were lower than that in the control group, and the levels of chemerin, RBP 4 and HOMA-β were higher than those of the control group, the difference was statistically significant (P<005);② Pearson correlation analysis showed that fasting blood glucose levels, fasting insulin levels, and HOMA-IR were positively correlated with chemerin and RBP 4 (P<005), but were negatively correlated with ADP (P<005);③ The sensitivity was the ordinate, and the 1- specificity was the abscissa,and the ROC curves of ADP, chemerin, RBP4 and three combined diagnoses were made respectively,which the sensitivity and specificity of combined detection of three groups in GDM diagnosis were the highest, respectively were 865 % and 879 %,and the area under the curve was 0911 (0835~0946).ConclusionThe IR is positively related to serum chemerin and RBP 4 levels in patients with GDM, and the level of ADP are negatively correlated, and the combined detection of the three has certain clinical value in the diagnosis of GDM.
