摘要:
目的比较玻璃化冻融D 3卵裂期胚胎当日解冻和提前解冻移植的结局。方法回顾性分析2014年1月至2016年3月在沈阳东方菁华医院进行的398例冻胚复苏移植周期的临床资料，根据解冻时间不同分为两组：过夜培养组（提前解冻过夜培养后移植，346例）；当日解冻组（当日解冻移植，52例）。又将上述两组根据解冻前胚胎评分情况分为3组：优胚组共293例（≥1个6细胞Ⅱ级胚胎）；混合胚组共86例（1个≥6细胞Ⅱ级胚胎+≥1 个一般质量胚胎）和一般质量胚胎组共19例。比较各组间的移植结局。结果优胚组和一般质量胚胎组中，过夜培养组种植率和临床妊娠率略高于当日解冻组，而混合胚组中当日解冻组的种植率和临床妊娠率略高于过夜培养组，差异均无统计学意义（P＞005）。结论D 3卵裂期胚胎提前解冻过夜培养并不能显著提高种植率和临床妊娠率，故在平常工作中，可根据患者自身情况以及实验室的工作安排，选择合适的解冻时间。
Abstract:
ObjectiveTo compare the results of thawing and early thawing transplantation on the same day of vitrified freeze-thaw D 3 cleavage stage embryos. MethodsA retrospective analysis of 398 cases of frozen embryo resuscitation from January 2014 to March 2016 in Shenyang Dongfang Jinghua Hospital was carried out.According to the defrosting time,the patients were divided into two groups according to the thawing time: the overnight culture group (migrated overnight after thawing, 346 cases); the day of thawing group (thaw thawing transplantation, 52 cases). The above two groups were divided into three groups according to the pre-freezing embryo score: 293 cases in the excellent embyo group (≥1 6-cell class II embryo); 86 cases in the mixed embryo group(1≥ 6-cell Ⅱ grade embryos +≥1 general mass embryo) and general mass embryos group(19 cases). Compared the graft outcomes between the two groups. ResultsIn the excellent embryo group and the general mass embryo group, the implantation rate and clinical pregnancy rate of the patients in overnight culture group were significantly higher than those of the day of thawing group patients. The implantation rate and clinical pregnancy rate of the thawed transplanted patients in the mixed embryo group were slightly higher than those of overnight culture group,but the difference was not statistically significant(P>005).ConclusionD 3 thawed embryo early thawing overnight culture can not significantly improve the planting rate and clinical pregnancy rate.Thus in the actual work,according to the patients' situation and the work schedule of the laboratory, choose the appropriate thawing time.
