摘要:
目的研究早孕期超声对胎儿解剖结构异常的评估价值。方法回顾性分析2015年2月至2017年2月在深圳市龙华区中心医院接受产检的5 000例孕妇的临床资料。对所有产妇行产前超声诊断,并详细记录、对比分析产妇早孕期超声检测与中晚孕期超声检测出的胎儿异常情况。结果产前超声检出83例结构异常胎儿,发生率为166 %。其中早孕期超声检测结果:胎儿水肿9例(1084 %),颈项透明层增厚10例(1205 %),露脑畸形2例(241 %),腹裂11例(1325 %),巨膀胱13例(1566 %),脐膨出11例(1325 %)。83例结构异常胎儿中晚孕期超声检测结果:面部畸形7例(843 %),心脏异常1例(120 %),脑积水6例(723 %),中枢神经异常9例(1084 %),泌尿系统异常2例(241 %),腹腔积液2例(241 %)。产前超声结构异常胎儿早孕期超声检出率(6747 %,56/83)高于中晚孕期超声检出率(3253 %,27/83),差异有统计学意义(P<005)。结论早孕期超声检测在评估胎儿解剖结构中具有极其重要的价值,能有效显示胎儿解剖结构异常情况,有利于临床上针对性方案的制定实施。但该检测方式只能检出一部分早期、严重的结构异常,存在一定的局限性。
Abstract:
ObjectiveTo evaluate the value of ultrasound in early pregnancy on fetal abnormal anatomy.MethodsThe clinical data of 5,000 pregnant women who were examined at the Central Hospital of Longhua District,Shenzhen from February 2015 to February 2017 were retrospectively analyzed. Prenatal ultrasound diagnosis was performed for all women, and detailed analysis and comparative analysis of maternal ultrasound during early pregnancy and fetal abnormalities detected by ultrasound during mid-late pregnancy.ResultsThe results of prenatal ultrasound detection of 83 cases of structural abnormalities in early pregnancy were as follows:the detection rate of fetal edema was 1084 % (9/83), the detection rate of cervical transparent layer thickening was 1205 % (10/83), and the detection rate of exencephaly was 241 % (2/83). The detection rate of gastroschisis was 1325% (11/83), the detection rate of megabladder was 1566 % (13/83), and the detection rate of pcromphalus was 1325% (11/83). Ultrasound detection of 83 cases of structural abnormalities in mid-late pregnancy were as follows: the detection rate of facial malformation was 843 % (7/83), the fetal detection rate of cardiac abnormalities was 120 % (1/83), and the detection rate of hydrocephalus was 723 % (6/83). The detection rate of central nervous system abnormalities was 1084% (9/83), the fetal detection rate of urinary system abnormalities was 241 % (2/83), and the fetal detection rate of peritoneal effusion was 241 % (2/83). Prenatal ultrasound detected 83 cases of structural abnormalities. The early detection rate of fetal ultrasound was 6747 % (56/83), which was significantly higher than that of 3253 % (27/83) in the middle and late pregnancy, the difference was statistically significant. (P<005). ConclusionUltrasound testing in early pregnancy has extremely important value in evaluating fetal anatomy, effectively showing fetal abnormal anatomy, thus facilitating the formulation and implementation of clinically targeted programs. However, this detection method can only detect a part of early and serious structural anomalies, and there are still some limitations.
