摘要:
目的探讨影响供精人工授精（artificial insemination with donor semen，AID）临床妊娠率的相关因素。方法收集2015年10月至2016年12月河北省计划生育科学技术研究院优生指导中心实施自然周期宫腔内AID的不孕夫妇1 500对的临床资料。分析女方年龄、不孕年限、不孕类型、输卵管通畅情况、子宫内膜厚度、子宫内膜分型、助孕方案等与AID临床妊娠率的关系。结果随着女方年龄增加、不孕年限增加，AID临床妊娠率呈下降趋势（P＜005）；继发不孕患者AID临床妊娠率高于原发不孕患者（P＜005）；A型子宫内膜AID的临床妊娠率高于其他类型子宫内膜（P＜005）；双侧输卵管通畅与单侧输卵管通畅、子宫内膜厚度、助孕方案的选择、各手术方式对AID临床妊娠率的影响不明显（P＞005）；一个治疗周期中，行2次AID手术治疗的临床妊娠率优于行1次AID手术治疗（P＜005）。结论AID是一种有效安全的助孕手段，女方年龄、不孕年限、不孕类型、子宫内膜类型等是影响AID临床妊娠率的重要因素。
Abstract:
ObjectiveTo investigate the factors affecting the clinical pregnancy rate of artificial insemination by donor (AID). MethodsThe clinical data of infertile couples who performed natural cycle intrauterine AID from October 2015 to December 2016 in Eugenics Guidance Center of Hebei Province Family Planning Science and Technology Research Institute were collected. Analyzed the relationship between the age of the woman, the age of infertility, the type of infertility, the patency of the fallopian tube, the endometrial thickness, the endometrial classification, the pregnancy-supporting program and the clinical pregnancy rate of AID. ResultsWith the increase of female age and infertility time, the clinical pregnancy rate of AID operation showed a decreasing trend (P<005). The clinical pregnancy rate of AID in secondary infertility patients was higher than that in primary infertility patients (P<005); the clinical pregnancy rate of type A endometrial AID was higher than other types of endometrium (P<001). Bilateral fallopian tube patency and unilateral fallopian tube patency, endometrial thickness, choice of assisted pregnancy program, and different surgical methods have no significant effect on AID clinical pregnancy rate (P>005). In one treatment cycle, the clinical pregnancy rate of 2 times AID surgery was better than that of one time AID surgery (P<005).ConclusionAID is an effective and safe means of assisting pregnancy. The age of the woman, the age of infertility, the type of infertility, and the type of endometrium are important factors influencing the clinical pregnancy rate of AID.
