摘要:
目的分析不同分娩方式对分娩后3~7 d乳汁成分的影响。方法选择2016年12月至2017年12月在四川省妇幼保健院进行剖宫产及顺产的产妇各100名,分别设为剖宫产组和阴道分娩组。收集分娩后3~7 d的乳汁,测定、比较母乳中主要营养成分的含量。结果阴道分娩组乳汁中脂肪[(322±112)g/dL]、蛋白质[(295±131)g/dL]、干物质含量[(1213±141)g/dL]及总能量[(6550±615)kcal/dL]明显高于剖宫产组[(243±111)g/dL]、[(246±058)g/dL]、[(1170±137)g/dL]、[(5998±991)kcal/dL],差异有统计学意义(P<005);而两组糖类含量[(611±071)g/dL]vs[(620±069)g/dL]比较差异无统计学意义(P>005);应用多重线性回归分析显示分娩方式是3~7 d乳汁中脂肪、蛋白质、干物质及总能量的主要影响因素(P<005);而年龄、身高、分娩方式、新生儿性别和出生体重均对糖类没有影响(P>005)。结论阴道分娩产妇3~7 d乳汁成分明显优于剖宫产产妇,应积极鼓励阴道分娩。
Abstract:
ObjectiveTo investigate the effects of different delivery modes on the composition of human colostrum 3~7 d post delivery. MethodsFrom December 2016 to December 2017, 200 maternal were placed in Sichuan Provincial Hospital for Women and Children. They were named caesarean section group(100) and vaginal delivery group(100) respectively.Colostrum was collected 3 to 7 days after delivery, and the content of major nutrients in breast milk was determined and compared. ResultsIn the vaginal delivery group, the fat [(322±112)g/dL], protein[(295±131)g/dL], dry matter content[(1213±141)g/dL], and total energy[(6550±615)kcal/dL] in colostrum were significantly higher than those in the cesarean section [(243±111)g/dL,(246±058)g/dL, (1170±137)g/dL, (5998±991)kcal/dL](P<005);There was no significant difference in sugar content between the two groups[(611±071)g/dL vs (620±069)g/dL](P>005).Multiple linear regression analysis showed that the mode of delivery was the main influencing factor of fat, protein, dry matter and total energy in colostrum (P<005);the age, height, mode of delivery, sex at birth, and birth weight had no effect on carbohydrates(P>005).ConclusionThe composition of colostrum 3~7 d post delivery in vaginal delivery is obviously higher than that in cesarean section. Vaginal delivery should be actively encouraged.
