摘要:
目的探讨经阴道三维容积超声在多囊卵巢综合征(polycystic ovarian syndrome,PCOS)中的影像特点及诊断价值。方法选择三二一医院2016年12月至2017年12月收治的PCOS患者57例及同期健康体检的女性50例为研究对象,对比分析经阴道三维容积超声在PCOS中的影像特点及诊断价值。结果经阴道三维容积超声检查,PCOS组的卵巢间质内血管数量较为丰富,且清晰可见,血流显示率为9164 %,并有1支较大的纵向贯穿卵巢间质的血管;而健康组的卵巢间质内显示血流信号较弱,表现为稀疏、散在点状,血流显示率为8168 %。PCOS组的睾酮、雌激素、黄体生成素及卵泡刺激素均高于健康组;PCOS组的卵巢容积、卵泡容积、卵泡数量、间质容积分别为(1324±324)cm3、(112±060)cm3、(1516±492)个、(867±305)cm3,均多于健康组[(601±290)cm3、(034±019)cm3、(381±192)个、(470±199)cm3]。PCOS组的阻力指数(059±009)小于健康组(093±008),收缩期峰值血流速度、舒张末期血流速度分别为(1284±236)cm/s、(672±136)cm/s,均快于健康组[(901±304)cm/s、(519±110)cm/s],以上各项结果比较差异均有统计学意义(P<005)。结论经阴道三维容积超声可通过计算卵巢的容积参数及血流参数,作为诊断PCOS的客观定量检查,提高PCOS的检出率,值得临床推广使用。
Abstract:
ObjectiveTo research imaging characteristics and diagnostic value of transvaginal three-dimensional volume ultrasound in polycystic ovary syndrome (PCOS). Methods57 patients with PCOS and 50 women with health examination during the same period from December 2016 to December 2017 in San Er Ling Yi Hospital were selected as the subjects. To compare the imaging features and diagnostic value of transvaginal three-dimensional volume ultrasound in PCOS. ResultsTransvaginal three-dimensional volume ultrasound showed that the number of vessels in the ovarian stroma of the PCOS group was abundant and clearly visible, showing a blood flow rate of 9164 %, and a large longitudinal blood vessel passing through the ovarian interstitium. In the healthy group, the blood flow in the ovarian stroma was sparse, scattered and punctate, and the blood flow signal was weak. The blood flow rate was 8168 %, and the difference was statistically significant (P<005). The testosterone, estrogen, luteinizing hormone and follicle stimulating hormone in the PCOS group were significantly higher than those in the healthy group, the difference was statistically significant (P<005). The ovarian volume, follicular volume, follicle volume and interstitial volume in the PCOS group were (1324 + 324) cm3, (112 + 060) cm3, (1516 + 492), and (867 + 305) cm3, respectively, which were all more than those in the healthy group[(601±290)cm3、(034±019)cm3、(381±192)、(470±199)cm3], the difference was statistically significant (P<005) Comprehensive resistance index of PCOS group was (059 + 009), significantly less than the healthy group(093±008), the peak systolic flow velocity, the end diastolic velocity were (1284 + 236) cm/s, (672 + 136) cm/s, were faster than the healthy group[(901±304)cm/s、(519±110)cm/s], the differences were statistically significant (P<005). ConclusionTransvaginal three-dimensional volume ultrasound can be used to calculate ovarian volume parameters and blood flow parameters, which can be used as an objective quantitative examination for diagnosing PCOS, and improve the detection rate of polycystic ovary syndrome. It is worth popularizing widely in clinic.
