摘要:
目的研究配对盒基因家族成员1(Pax 1)和LIM同源盒转录因子1α(LMX 1A)在宫颈人乳头瘤病毒(human papillomavirus,HPV)持续感染患者脱落细胞中的表达关系及其临床意义。方法收集2015年5月至2019年1月上海市闵行区中心医院收治的高危亚型HPV持续感染者150例为研究对象,根据阴道镜活检后病理学诊断分4组:慢性宫颈炎症组(对照组)35例,宫颈低级别上皮内瘤变(LG-CIN组)40例,宫颈高级别上皮内瘤变(HG-CIN组)42例,宫颈鳞癌(SCC组)33例,收集患者宫颈脱落细胞,采用实时定量PCR(RT-qPCR)检测Pax 1、LMX 1A mRNA表达水平,采用免疫印迹检测Pax 1和LMX 1A蛋白表达水平;采用二代杂交捕获法检测HPV DNA负荷量,采用Pearson法分析其相关性;采用Logistic回归分析影响宫颈癌发生的危险因素。结果与对照组比较,LG-CIN组、HG-CIN组、SCC组患者宫颈脱落细胞中Pax 1、LMX 1A mRNA及蛋白水平降低,HPV-DNA负荷量升高(P均<005);与LG-CIN组比较,HG-CIN组、SCC组患者宫颈脱落细胞中Pax 1、LMX 1A mRNA及蛋白水平降低,HPV-DNA负荷量升高(P均<005);与HG-CIN组比较,SCC组患者宫颈脱落细胞中Pax 1、LMX 1A mRNA及蛋白水平降低,HPV-DNA负荷量升高(P均<005)。Pearson分析结果显示,宫颈癌患者宫颈脱落细胞中Pax 1、LMX 1A蛋白表达水平呈负相关(P<005),Pax 1、LMX 1A蛋白表达水平分别与高危HPV-DNA负荷量呈负相关(P<005)。Pax 1、LMX 1A蛋白水平高是影响宫颈癌发生的独立保护因素(P均<005),高危HPV-DNA负荷量高是影响宫颈癌发生的独立危险因素(P<005)。结论宫颈脱落细胞中Pax 1、LMX 1A蛋白表达下调,与高危HPV-DNA负荷量呈负相关,二者高表达是影响宫颈癌发生的独立保护因素,可能成为宫颈病变进展监测的新标志分子。
Abstract:
ObjectiveTo study the expressions of paired box gene family member 1 (Pax 1) and LIM homeobox transcription factor 1 alpha (LMX 1A) in exfoliated cells of patients with persistent cervical human papillomavirus (HPV) infection and their clinical significances. Methods150 cases of high-risk subtype HPV persistent infection were collected from May 2015 to January 2019 in Minhang District Central Hospital of Shanghai City. According to the pathological diagnosis based on colposcopy biopsy, they were divided into 4 groups: 35 cases of chronic cervicitis (control group), 40 cases of low-grade cervical intraepithelial neoplasia (LG-CIN group), 42 cases of high-grade cervical intraepithelial neoplasia (HG-CIN group) and 33 cases of cervical squamous cell carcinoma (SCC group). The cervical exfoliated cells were collected, the expression levels of Pax 1 and LMX 1A mRNAs were detected by real-time quantitative PCR (RT-qPCR), the expression levels of Pax 1 and LMX 1A proteins were detected by immunoblotting. HPV DNA load was detected by second generation hybridization capture method, Pearson method was used to analyze the correlation, and Logistic regression was used to analyze the risk factors of cervical cancer. ResultsCompared with the control group, the levels of Pax 1, LMX 1A and protein in cervical exfoliated cells of LG-CIN group, HG-CIN group and SCC group decreased, and the load of HPV-DNA increased (P<005). Compared with LG-CIN group, the levels of Pax 1, LMX 1A and protein in cervical exfoliated cells of HG-CIN group and SCC group decreased, and the HPV-DNA load increased (P<005). Compared with HG-CIN group, the levels of Pax 1, LMX 1A and protein in cervical exfoliated cells in SCC group decreased, and the HPV-DNA load increased (P<005). Pearson analysis showed that the expression levels of Pax 1 and LMX 1A proteins in cervical exfoliated cells of cervical cancer patients were negatively correlated (P<005), while the expression levels of Pax 1 and LMX 1A proteins were negatively correlated with the high-risk HPV-DNA load (P<005). High levels of Pax 1 and LMX 1A proteins were independent protective factors for cervical cancer (P<005), and high load of high-risk HPV-DNA was an independent risk factor for cervical cancer (P<005). ConclusionThe expressions of Pax 1 and LMX 1A in cervical exfoliated cells are down-regulated, which is negatively correlated with high-risk HPV-DNA load. The high expressions of Pax 1 and LMX 1A are independent protective factors affecting the occurrence of cervical cancer and may become new markers for monitoring the progress of cervical lesions.
