摘要:
目的检测复发性自然流产(recurrent spontaneous abortion,RSA)患者胎盘绒毛中CD 38、CD 138的表达情况,探讨二者在RSA中的作用。方法选取2017年4月至2019年5月东莞康华医院105例RSA患者为研究对象(RSA组),正常早孕人工流产者100例为对照组。收集两组一般资料,免疫组化检测胎盘绒毛中CD 38、CD 138水平,酶联免疫吸附(ELISA)法检测血清中辅助T细胞因子干扰素(interferon,INF)γ、肿瘤坏死因子(tumor necrosis factor,TNF)α、白介素(interleukin,IL)6、IL10、IL17表达水平。Pearson法分析CD 38、CD 138与IL1、IL6、IL10、INFγ、TNFα的相关性,Logistic分析影响RSA的因素。结果两组年龄、体质量指数、妊娠时间、孕囊大小、孕次、产次比较,差异均无统计学意义(P>005)。RSA组胎盘绒毛中CD 38、CD 138阳性比例,血清中INFγ、TNFα、IL17高于对照组(P<005);血清中IL6、IL10低于对照组(P<005)。Pearson法分析,CD 38、CD 138与IL6、IL10呈负相关(P均<005),与TNFα呈正相关(P均<005)。CD 138与IL17呈正相关(P<005)。Logistic分析显示,CD 38、CD 138、TNFα是影响RSA的危险因素,IL6、IL10是影响RSA的保护因素;CD 38、CD 138是影响RSA的独立危险因素,IL10是影响RSA的独立保护因素。结论RSA胎盘绒毛中CD 38、CD 138高表达,与血清中辅助T细胞因子关系密切,可能与辅助T细胞因子共同作用影响RSA的发生。
Abstract:
ObjectiveDetection of CD 38, CD 138 expression in placental chorionic villus of patients with recurrent spontaneous abortion (RSA) and exploration of the role of both in RSA.Methods105 cases of RSA patients from April 2017 to May 2019 in Dongguan Conghua Hospital were selected as study subjects, and 100 cases of normal early pregnancy abortion were used as controls. Two sets of general data were collected, immunohistochemistry was used to detect CD 38 and CD 138 levels in placental chorionic villus, enzymelinked immunosorbent assay (ELISA) was used to detect the expression levels of cofactor T cytokine interferon (INF)γ, tumor necrosis factor (TNF)α, interleukin (interleukin,IL)6, IL10, IL17 in serum. Pearson method was used to analyze the correlation between CD 38, CD 138 and IL1, IL6, IL10, INFγ, TNFα, Logistic analysis was used to analyze factors affecting RSA.ResultsThe differences of age, body mass index, gestation time, gestation bladder size, gestation time and birth time of the two groups were not statistically significant (P>005).The proportion of placental chorionic villus with positive CD 38 and CD 138 and serum INFγ, TNFα,IL17 level was higher in the RSA group than in the control group (P<005), while serum IL6 and IL10 were lower than in the control group (P<005).Pearson's method showed that CD 38 and CD 138 were negatively associated with IL6 and IL10 (P<005) and positively associated with TNFα (P<005).CD 138 was positively associated with IL17 (P<005).Logistic analysis showed that CD 38, CD 138 and TNFα were risk factors for RSA and IL6 and IL10 were protective factors for RSA; CD 38 and CD 138 were independent risk factors for RSA and IL10 was independent protective factor for RSA.ConclusionThe high expression of CD 38 and CD 138 in the placental chorion of RSA is closely related to the helper T cytokines in the serum, which may coact with helper T cytokines to influence the occurrence of RSA.
