摘要:
目的探讨网膜素与瘦素在子痫前期患者血清中的表达及意义。方法2016年3月至2018年3月在松滋市人民医院住院及门诊产检的孕32~37周的单胎首次妊娠孕妇中随机抽取40例轻度子痫前期患者,设为轻度子痫前期(mild preeclampsia,MPE)组;40例重度子痫前期(early on severe preeclampsia,EOSP)患者设为EOSP组;40例健康孕妇设为对照组。比较各组血清中总胆固醇(total cholesterol, TC)、甘油三酯(triglyceride, TG)、高密度脂蛋白(highdensity lipoprotein, HDLC)、低密度脂蛋白(lowdensity lipoprotein, LDLC)、空腹胰岛素(fasting insulin, FINS)、网膜素、瘦素水平,计算HOMAIR。Pearson直线相关分析,统计网膜素与TG、TC、HDLC、LDLC、FINS、HOMAIR、瘦素之间的相关性。采用Logistic多因素回归模型筛选影响子痫前期病情严重程度的影响因素。结果MPE组、EOSP组TG、TC、LDLC、FINS、HOMAIR、瘦素均明显高于对照组,HDLC、网膜素均明显低于对照组,差异有统计学意义(P<005);EOSP组LDLC、FINS、HOMAIR、瘦素均明显高于MPE组,HDLC、网膜素均明显低于MPE组,差异有统计学意义(P<005);网膜素水平与TG、TC、LDLC、FINS、HOMAIR、瘦素均呈显著负相关(P<005),与HDLC呈显著正相关(P<005)。FINS、HOMAIR、瘦素为重度子痫前期的独立危险因素,网膜素为重度子痫前期的保护性因素。结论网膜素水平降低、瘦素水平升高与脂代谢紊乱及子痫前期的病情加重有关。
Abstract:
ObjectiveTo explore the expression and significance of omentin and leptin in the serum of patients with preeclampsia.MethodsFrom March 2016 to March 2018, 40 patients with mild preeclampsia (MPE) were randomly selected from the pregnant women with single pregnancies of 32 to 37 weeks of gestation in Songzi People's Hospital and outpatient obstetrics and set as MPE group; 40 patients with early on severe preeclampsia (EOSP) were set as EOSP group; 40 healthy pregnant women were set as control group. Compared the serum total cholesterol (TC), triglyceride (TG), highdensity lipoprotein (HDLC), lowdensity lipoprotein (LDLC), fasting insulin (FINS), omentin and leptin in each group, calculated HOMAIR. Pearson linear correlation analysis was used to analyze statistical correlation between omentin and TG, TC, HDLC, LDLC, FINS, HOMAIR, leptin. Logistic multifactor regression model was used to screen the factors that affected the severity of preeclampsia.ResultsTG, TC, LDLC, FINS, HOMAIR, and leptin in the MPE group and EOSP group were significantly higher than that in the control group, and HDLC and omentin were significantly lower than the control group, the differences were statistically significant (P<005); LDLC, FINS, HOMAIR and leptin in the EOSP group were significantly higher than those in the MPE group, HDLC and omentin were significantly lower than the MPE group, the differences were statistically significant (P<005); Omentin levels were significantly negatively correlated with TG, TC, LDLC, FINS, HOMAIR, and leptin (P<005), and significantly positively correlated with HDLC (P<005). FINS, HOMAIR and leptin were independent risk factors for severe preeclampsia, and omentin was a protective factor for severe preeclampsia.ConclusionDecreased omentin level and increased leptin level are related to the disorder of lipid metabolism and the aggravation of preeclampsia.
