摘要:
目的探讨胎儿先天性血管环的超声图像特征及意义。方法回顾性分析2018年1~12月在四川省妇幼保健院超声医学科行产前超声筛查的20 356例孕妇资料，其中24例诊断为先天性血管环，分析该24例胎儿先天性血管环的图像特征，并随访观察胎儿血管环类型、合并其他畸形情况、染色体情况、孕妇妊娠结局及产后患儿情况。结果产前超声筛查结果显示，24例胎儿血管环中，9例为完全型血管环：其中2例形成“O”形：双主动脉弓，7例形成“U”形（6例右位主动脉弓、左动脉导管伴迷走左锁骨下动脉；1例右位主动脉弓、双动脉导管伴迷走左锁骨下动脉）；15例为不完全型血管环：其中12例形成“C”形：左位主动脉弓、左动脉导管伴迷走右锁骨下动脉，3例肺动脉吊带。随访结局：20例染色体核型正常，孕妇继续妊娠，出生后均暂未出现明显压迫气管和食管症状，4例因合并畸形或染色体核型异常，孕妇引产终止妊娠。结论详细的产前超声检查对胎儿先天性血管环的筛查准确率非常高，染色体核型正常且未合并其他心血管畸形者妊娠结局良好，否则可能导致不良结局。
Abstract:
ObjectiveTo explore the image characteristics and significance of fetal congenital vascular ring. MethodsA retrospective analysis was made of 20 356 pregnant women who were screened by ultrasound from January to December 2018 in the Department of Ultrasound Medicine of Sichuan Provincial Maternity and Child Health Care Hospital.24 cases were diagnosed as congenital vascular rings. The image characteristics of them were analyzed. The types of fetal vascular rings, the complications of other malformations, chromosome conditions and pregnancy outcomes were followed up. ResultsThe results of prenatal ultrasound screening showed that of the 24 fetal vascular rings, 9 cases were complete vascular rings:2 of them formed an “O” shape: double aortic arch; 7 cases formed a “U” shape (6 cases of right aortic arch, left arterial catheter with vagus left subclavian artery; 1 case with right aortic arch and double arterial catheter with vagus left subclavian artery); 15 cases with incomplete vascular ring: 12 cases formed “C” shape: left aortic arch, left artery Catheter with vagus right subclavian artery, 3 cases of pulmonary artery sling. Followup outcomes: 20 cases with normal karyotypes, pregnant women continued pregnancy, and no signs of oppression of trachea and esophagus appeared after birth. 4 cases of pregnancy induced abortion due to abnormalities or abnormal chromosome karyotypes.ConclusionThe accuracy of the screening of the fetal congenital vascular ring by detailed prenatal ultrasound examination is very high, and the pregnancy outcome is good for those with normal chromosome karyotype and no other cardiovascular malformations, otherwise it may lead to adverse outcomes.
