摘要:
目的探讨卵裂期胚胎细胞数与碎片对囊胚形成与移植结局的影响。方法回顾性分析2017年1月至2019年6月在河南省人民医院生殖医学中心行体外囊胚培养患者临床资料,根据卵裂期胚胎的细胞数分为A 1组(养囊胚胎数15 130个,移植周期数40个):胚胎细胞数≤7;A 2组(养囊胚胎数13 899个,移植周期数411个):胚胎细胞数8~10;A 3组(养囊胚胎数2 680个,移植周期数57个):胚胎细胞数≥11。根据卵裂期胚胎的碎片数分为B 1组(养囊胚胎数21 805个,移植周期数410个):胚胎碎片≤10 %;B 2组(养囊胚胎数9 904个,移植周期数98个):胚胎碎片>10 %。通过分析不同组别的可利用囊胚形成率、优质囊胚形成率、移植患者年龄、胚胎移植日内膜厚度、患者不孕年限和临床妊娠率,探讨卵裂期胚胎细胞数与碎片对囊胚形成及移植结局的影响。结果A 1组的可利用囊胚形成率(3280 %)低于A 2和A 3组(6767 %、6705 %)差异有统计学意义(P<005),A 2组与A 3组的可利用囊胚形成率比较,差异无统计学意义(P>005);A 1组、A 2组与A 3组的优质囊胚形成率比较,差异有统计学意义(P<005)。B 1组和B 2组的可利用囊胚形成率分别为6062 %、2976 %,优质囊胚形成率分别为1973 %、429 %,两组的可利用囊胚形成率、优质囊胚形成率比较,差异均有统计学意义(P<005)。A 1组、A 2组与A 3组之间,以及B 1组与B 2组之间的移植患者年龄、胚胎移植日内膜厚度、患者不孕年限、临床妊娠率比较,差异均无统计学意义(P>005)。结论卵裂期胚胎细胞数、碎片与囊胚形成的结局密切相关,但是对囊胚移植的结局没有显著的影响。
Abstract:
ObjectiveTo explore the effect of embryonic cell number and fragmentation at the cleavage stage on blastocyst formation and transplantation outcome.MethodsThe clinical data of patients undergoing in vitro blastocyst culture at the Reproductive Medicine Center of Henan Provincial People's Hospital from January 2017 to June 2019 were retrospectively analyzed. They were divided into group A 1 (number of cystic embryos 15 130,the number of transfer cycles was 40):the number of embryonic cells was ≤7, group A 2 (13 899 embryonated cysts, number of transfer cycles 411):8~10 embryonic cells, and group A 3 (2 680 embryonated embryos, number of transplantation cycles 57):embryo cell number ≥11. According to the fragmentation number of embryos in the cleavage stage, they were divided into group B 1 (21 805 embryonated cysts and 410 transfer cycles):embryo fragments≤10 %, and group B 2 (9 904 embryonated cysts and 98 transfer cycles):embryo fragments>10 %. By analyzing the available blastocyst formation rate, highquality blastocyst formation rate, age of transplanted patients, intimal thickness of embryo transfer day, infertility years and clinical pregnancy rate of different groups, Investigate the influence of the number of embryonic cells and fragments in the cleavage stage on blastocyst formation and transfer outcome.ResultsThe available blastocyst formation rates of A 1(328%)was lower than that in group A 2 and A 3(6767 %,67.05 %).The difference was statistically significant (P<005).There was no statistically significant difference in availability blastocyst formation rate between group A 2 and group A 3(P>005);comparison of highquality blastocyst formation rates in A 1,A 2 and A3 groups, the difference was statistically significant (P<005). The available blastocyst formation rates of the B 1 and B 2 groups were 6062 % and 2976 %, respectively, and the highquality blastocyst formation rates were 1973 % and 429 %, respectively.All have statistical significance (P<005).There were no statistically significant differences in the age of the transplanted patients, the intimal thickness of the embryo transfer day, the infertility years of the patients, and the clinical pregnancy rate between the A 1 group, A 2 group and A 3 group, and between the B 1 and B 2 groups (P>005).ConclusionThe number and fragments of embryonic cells in the cleavage stage are closely related to the outcome of blastocyst formation, but have no significant effect on the outcome of blastocyst transplantation.
