摘要:
目的探讨体质量指数(body mass index,BMI)联合血脂及脂溶性维生素水平预测不同类型重度子痫前期(preeclampsiam PE)的价值。方法回顾性分析2017年12月至2019年6月在淮北市人民医院建档产检诊断为重度 PE 孕妇62例的临床资料,根据孕妇重度 PE 类型分为早发型重度 PE 组(41例)和晚发型重度 PE 组(21例);并按照1∶2比例选取同期正常单胎产检孕妇124例,作为对照组,对比各组间的 BMI、血脂以及脂溶性维生素等指标,分析影响孕妇发生不同类型重度 PE 的影响因素,对 BMI 联合血脂及脂溶性维生素水平预测不同类型重度 PE 的价值进行探讨。结果研究组在 BMI、总胆固醇(total choles terol,TC)、甘油三酯(triglyceride,TG)、低密度脂蛋白(low density lipoprotein,LDL)、维生素A、维生素E 方面高于对照组,研究组在高密度脂蛋白(highdensity lipoprotein,HDL)、维生素D3方面低于对照组,差异均有统计学意义(P<005)。Logistic回归分析结果显示,BMI、TC、TG、高密度脂蛋白(highdensity lipoprotein,HDL)、LDL、维生素A、维生素D3、维生素 E 是影响早发型重度 PE 发生的独立因素(P<005);BMI、TC、TG、维生素D3、维生素E是影响晚发型重度 PE 发生的独立因素(P<005)。BMI 联合血脂及脂溶性维生素水平预测早发型重度 PE 的 ROC 曲线下面积为0855,敏感度为877 %,特异度为567 %,具有较高的预测价值。BMI 联合血脂及脂溶性维生素水平预测晚发型重度 PE 的ROC曲线下面积为0579,敏感度为0525,特异度为0549,预测价值较低。结论BMI、血脂及脂溶性维生素水平在不同类型重度 PE 孕妇中与正常孕妇有差异,BMI 联合血脂及脂溶性维生素水平预测早发型重度 PE 发病价值较高。
Abstract:
ObjectiveTo explore the value of body mass index (BMI) combined with blood lipid and fatsoluble vitamin levels in predicting different types of severe preeclampsia(PE).MethodsThe clinical data of 62 pregnant women diagnosed as severe PE with Huaibei People's Hospital from December 2017 to June 2019 were retrospectively analyzed. According to the type of severe PE of pregnant women, they were divided into earlyonset severe PE group (41 cases) and lateonset severe PE group (21 cases); 124 pregnant women with normal singletons were selected according to the ratio of 1∶2 at the same time, as a control group, BMI, blood lipid and fatsoluble vitamins and other indicators were compared between the groups to analyze the different types of pregnant women influencing factors of severe PE, to explore the value of BMI combined with blood lipid and fatsoluble vitamin levels to predict the value of different types of severe PE.ResultsThe study group was higher than the control group in BMI, TC, TG, LDL, vitamin A, and vitamin E. The study group was lower than the control group in highdensity lipoprotein (HDL) and vitamin D3. All had statistical significance (P<005). Logistic regression analysis results showed that BMI, TC, TG, HDL, low density lipoprotein (LDL), vitamin A, vitamin D3, and vitamin E were independent factors that affect the occurrence of earlyonset severe PE (P<005); BMI, TC, TG, vitamin D3, and vitamin E were independent factors that affect the occurrence of lateonset severe PE (P<005). BMI combined with blood lipid and fatsoluble vitamin levels predicts that the area under the ROC curve of earlyonset severe PE was 0855, the sensitivity was 877 %, and the specificity was 567 %, which had a high predictive value. The area under the ROC curve of BMI combined with blood lipid and fatsoluble vitamin levels to predict lateonset severe PE was 0579, sensitivity was 0525, specificity was 0.549, and the predictive value was low.ConclusionThe levels of BMI, blood lipids and fatsoluble vitamins in pregnant women with different types of severe PE are different from those of normal pregnant women. BMI combined with blood lipids and fatsoluble vitamin levels have high value of predicting earlyonset severe PE.
