摘要:
目的比较新鲜周期胚胎移植和冻融胚胎移植后妊娠早期胎儿生长发育的速度。方法回顾性分析2019年1~5月在唐都医院生殖医学中心行胚胎移植术后单胎妊娠的患者150例,其中新鲜周期胚胎移植70例,冻融周期胚胎移植80例,两组的年龄、体质量指数、不孕年限、受精方式方面匹配。比较两组孕妇妊娠早期(约孕13周)胎儿头臀径、双顶径、股骨长等生长指标及胎儿的发育速度,比较两组的胎盘厚度。结果与新鲜周期胚胎移植后妊娠早期胎儿相比,冻融胚胎移植后妊娠早期胎儿的头臀径[(7159±539) mm vs (6884±488)mm]、双顶径[(2340±148)mm vs(2267±141)mm]、股骨长度[(982±162)vs (902±117)mm]显著增大,各指标的生长速度显著增快,差异均有统计学意义(P<005);胎盘厚度[(1658±242)mm vs (1622±311)mm]和胎盘生长速度[(018±003) mm/d vs (018±003)mm/d]两组比较差异无统计学意义(P>005)。结论不同移植策略助孕后妊娠早期胎儿生长速度不同,冻融周期胚胎移植较新鲜周期胚胎移植后妊娠早期的胎儿生长速度显著升高,这可能与巨大儿的发生有关。
Abstract:
ObjectiveTo compare the speed of fetal growth and development in early pregnancy after fresh cycle embryo transfer and freezethaw embryo transfer.MethodsA retrospective analysis of 150 cases of single pregnancy after embryo transfer in the reproductive medical center of Tangdu Hospital from January to may 2019, including 70 cases of fresh cycle embryo transfer and 80 cases of freezethaw cycle embryo transfer. The age, body mass index, infertility years and fertilization methods of the two groups were matched. The growth indexes of fetal head hip diameter, biparietal diameter, femur length and fetal development speed were compared between the two groups in the early pregnancy (about 13 weeks), and the placental thickness of the two groups was compared.ResultsCompared with the fetuses in the first trimester after fresh cycle embryo transfer, the crownrump length[(7159±539) mm vs (6884±488)mm], biparietal diameters[(2340±148)mm vs(2267±141)mm] and femur diameters [(982±162)vs (902±117)mm]of the fetuses after freezingthawed embryo transfer were significantly increased. Growth rates of these indexes indicate similar trends(P<005).However, there were no significant differences between the two groups in placental thickness [(1658±242 ) mm vs (1622±311 ) mm]and placental growth rate[(018±003) mm/d vs (018±003) mm/d] (P>005).ConclusionThe growth rate of fetus in early pregnancy after freezethaw embryo transfer is faster than that of fresh embryo transfer,which may be related to the occurrence of giant babies.
