摘要:
目的探讨糖化血红蛋白(glycosylated hemoglobin, HbA1c)、糖化血清蛋白(glycated serum protein, GSP)、C反应蛋白(C-reactive protein,CRP)联合检测对早期妊娠期糖尿病(gestational diabetes mellitus,GDM)的诊断价值。方法选取2013年9月至2016年10月东莞康华医院收治的110例GDM患者为GDM组,糖耐量受损孕妇110例为糖耐量减低(impaired glucose tolerance, IGT)组,同期健康孕妇100例为对照组。比较3组患者的糖筛查试验、HbA1c、GSP、CRP血清水平,3个指标检测异常率及诊断学指标。结果GDM组患者的糖筛查试验、HbA1c、GSP、CRP血清水平均显著高于IGT组和对照组(P<005)。GDM组HbA1c、GSP、CRP检测异常率均显著高于IGT组(P<005)。在单独及联合检测中,以HbA1c+GSP+CRP 3个指标联合检测的灵敏度和特异度最高(P<005)。结论GSP、HbA1c、CRP 3个指标联合检测对GDM具有较高的诊断价值。
Abstract:
ObjectiveTo investigate the value of glycosylated hemoglobin (HbA1c), glycated serum protein(GSP) and C-reactive protein(CRP) combined detection in the diagnosis of early gestational diabetes mellitus(GDM). MethodsFrom September 2013 to October 2016, 110 patients with GDM in Dongguan Kanghua Hospital were selected as GDM group, 110 cases of impaired glucose tolerance were selected as IGT group and 100 healthy pregnant women were selected as control group. The glucose screening test, HbA1c, GSP, CRP serum levels、abnormal rate of detection of three indexes、the diagnostic indexes of GDM by single and combined detection were compared between the three groups. ResultsThe glucose screening test, HbA1c, GSP, CRP serum levels of the GDM group were significantly higher than those in the IGT group and the control group(P<005); HbA1c, GSP, CRP abnormal rate of GDM group was significantly higher than that of the IGT group(P<005); the sensitivity and specificity of combined detection of HbA1c, GSP, CRP indexes were the highest in the single and combined detection(P<005). ConclusionGSP, HbA1c and CRP have important significance in the early diagnosis of GDM. Joint detection of the three indicators has high diagnostic value for GDM, and can be used as an auxiliary method for early diagnosis of GDM.
