摘要:
目的探究孕早期甲状腺素水平对妊娠期糖尿病(gestational diabetes mellitus,GDM)发病的影响。方法选取2016年6月至2017年6月于无锡市第九人民医院产科就诊的39例单纯甲状腺过氧化物酶抗体(thyroid peroxidase antibody,TPOAb)阳性孕妇(单纯TPOAb阳性组)、32例单纯低甲状腺素血症孕妇(单纯低甲状腺素血症组)和79例甲状腺功能正常孕妇(正常组),所有孕妇在11~13+6周行甲状腺功能检测,24~28周行口服葡萄糖耐量实验,检测对比3组孕妇促甲状腺素(thyroid stimulating hormone,TSH)、游离甲状腺素(free thyroxine,FT4)和TPOAb含量,及FT4与糖代谢的相关性和TPOAb对GDM的影响。结果单纯低甲状腺素血症组FT4含量低于正常组而TSH含量高于正常组(P<005),单纯低甲状腺素血症组患者1 h和2 h后血糖含量均高于正常组,差异有统计学意义(P<005);单纯低甲状腺素血症组和单纯TPOAb阳性组GDM患病率均高于正常组;TPOAb≥500 IU/mL时,GDM患病率比正常组高,差异有统计学意义(P<005)。FT4与空腹血糖、1 h血糖、2 h血糖呈负相关,FT4有利于降低GDM患病风险。结论妊娠早期单纯低甲状腺素血症和高浓度TPOAb都会增加GDM的发病率。
Abstract:
ObjectiveTo investigate the effect of thyroid hormone levels in early pregnancy on the onset of gestational diabetes mellitus(GDM). Methods39 pregnant women with thyroid peroxidase antibody (TPOAb)-positive (TPOAb positive group),32 pregnant women with low thyroid (simple hypothyroxinemia group) and 79 pregnant women of normal thyroid function (normal group) were selected in the Department of Obstetrics of the Ninth People's Hospital of Wuxi from June 2016 to June 2017.All pregnant women performed thyroid function tests at 11 to 13+6 weeks and oral glucose tolerance tests at 24 to 28 weeks. Women of three groups were tested for thyroid stimulating hormone (TSH) , free thyroxine (FT4) and TPOAb content, and the correlation between FT4 and glucose metabolism and the effect of TPOAb on GDM were analyzed. ResultsThe levels of FT4 in patients of simple hypothyroxinemia group were lower than those in the normal group and the levels of TSH were higher than those in the normal group, the levels of glucose 1 and 2 h after glucose tolerance test were higher than those in the normal group (P<005).The prevalence of GDM was higher in the simple hypothyroxine group and TPOAb positive group than in the normal group. The prevalence of GDM was higher in the patients with TPOAb ≥500 IU/mL than in the normal group (P<005). FT4 was negatively correlated with fasting blood glucose, 1 h blood glucose, and 2 h blood glucose. FT4 was beneficial in reducing the risk of GDM. ConclusionIn early pregnancy, hypothyroxineemia and high concentrations of TPOAb increase the incidence of GDM.
