摘要:
目的探讨子宫内膜容受性标志物(endometrial receptivity markers, ERM)在围着床期的表达与反复种植失败(recurrent implantation failure, RIF)的相关性。方法选择上海交通大学医学院附属仁济医院2016年5月至2017年5月行体外受精-胚胎移植(in vitro fertilization-embryo transfer, IVF-ET)患者共375例,采用ELISA检测195例RIF患者(RIF组)及180例成功妊娠患者(Control组)围着床期及增生期ERM的表达,进行组内及组间比较,分析围着床期ERM与RIF的相关性。结果围着床期RIF组与Control组相比,生长调节癌基因(growth-regulated oncogene α, GROα)、粒细胞巨噬细胞集落刺激因子(granulocyte-macrophage colony stimulating factor, GM-CSF)、白细胞介素-1β(interleukin-1β, IL-1β)、IL-6、干扰素诱导蛋白10(interferon - induced protein-10, IP-10)表达显著增高;IL-12、IL-18、IL-1受体拮抗剂(IL-1 receptor antagonist, IL-1Ra)、血管内皮细胞生长因子(vascular endothelial growth factor, VEGF)、白血病抑制因子(leukemia inhibitory factor,LIF)表达显著降低,差异均有统计学意义(P<005);IL-8及单核细胞趋化蛋白-1(monocyte chemotacticprotein 1, MCP-1)的表达在两组之间差异无统计学意义(P>005)。增生期RIF组与Control组相比,GROα、GM-CSF、IL-1β、IL-6、IP-10表达同样较高,VEGF和LIF表达显著性降低,两组比较差异均有统计学意义(P<005);IL-12、IL-18、IL-1Ra、IL-8及MCP-1在两组比较差异无统计学意义(P>005)。RIF组内比较,GROα、IP-10、IL-12、IL-18及VEGF在围着床期的表达显著高于增生期,Control组IL-12、IL-18、IL-1Ra、VEGF和LIF在围着床期的表达显著高于增生期。Spearman相关性分析显示,GROα、IP-10、IL-1Ra及LIF与RIF具有相关性。结论围着床期患者子宫内膜GROα和IP-10的表达增高及IL-1Ra和LIF的表达降低与RIF发生具有一定的相关性。
Abstract:
ObjectiveTo explore the relationship between endometrial receptivity markers (ERM) expression in the peri-implantation period and recurrent implantation failure (RIF). MethodsA total of 375 patients undergoing in vitro fertilization - embryo transfer (IVF-ET) from May 2016 to May 2017 in Renji Hospital affiliated to Shanghai Jiaotong University School of Medicine were selected.ELISA was used to detect the expression of ERM in 195 cases of RIF patients (RIF group) and 180 cases of successful pregnant patients (Control group) in peri-implantation period and proliferative period.The intra- and inter-group comparisons were performed to analyze the correlation between ERM during the peri-implantation period and RIF. ResultsCompared to control group during the peri-implantation period, growth-regulated oncogene α (GROα), granulocyte-macrophage colony stimulating factor (GM-CSF), interleukin-1β (IL-1β), IL-6, interferon - induced protein-10 (IP-10) were significantly increased and IL-12, IL-18, IL-1 receptor antagonist (IL-1Ra), vascular endothelial growth factor (VEGF), leukemia inhibitory factor (LIF) were significantly decreased in RIF group, the differences were statisticauy significant(P>005).There were no significant differences of IL-8 and monocyte chemotacticprotein 1 (MCP-1) between two groups(P>005). Compared with control group in the widow of hyperplasia, GROα, GM-CSF, IL-1β, IL-6 and IP-10 were all significantly increased and VEGF and LIF were both significantly decreased in RIF group, the differences were statisticauy significant(P>005). There were no significant differences of IL-12, IL-18, IL-1Ra, IL-8 and MCP-1 between two groups(P>005). In RIF group, the expressions of GROα, IP-10, IL-12, IL-18 and VEGF were higher in the widow of implantation than that in the widow of hyperplasia, in control group, the expressions of IL-12, IL-18, IL-1Ra, VEGF and LIF were higher in the peri-implantation period than that in the proliferative. Spearman correlation analysis showed that GROα, IP-10, IL-1Ra and LIF were associated with RIF. ConclusionThe increased expression of GROα and IP-10, the decreased expression of IL-1Ra and LIF of patients in the widow of implantation have a certain correlation with RIF.
