摘要:
目的探讨Cajal间质细胞(interstitial cells cajac,ICC)在输卵管妊娠中的表达和临床意义，为异位妊娠的早期诊断和发病机制研究提供新思路。方法选取2016~2017年宜昌市中心人民医院妇科收治的输卵管切除患者152例作为研究对象，其中88例因输卵管异位妊娠切除输卵管患者为研究组，64例因妇科疾病切除输卵管患者为对照组。采用免疫组化及Western Blot法分别检测患者输卵管中ICC的表达，比较两组ICC数量和c-kit蛋白表达水平差异，分析其与临床指标的相关性。结果免疫组化中，研究组c-kit阳性细胞数目明显少于对照组，Western blot检测中，研究组c-kit蛋白比值较对照组低，差异均有统计学意义（P＜005）。两组患者在是否有流产史，以及血β-人绒毛膜促性腺激素（human chorionic gonadotrophin beta，β-hCG）、孕激素、肌酸激酶（creatine kinase，CK）、血管内皮生长因子（vascular endothelial growth factor，VEGF）、白细胞介素（interleukin,IL）-6、IL-8、肿瘤坏死因子α（tumor necrosis factor alpha,TNF-α）和激活素A等各项指标比较差异有统计学意义（P＜005）。152例患者的ICC表达与血β-hCG、CK、VEGF和激活素A存在相关性（P＜005）。结论ICC在输卵管妊娠中表达减少，可能导致输卵管运动能力降低，增加异位妊娠发生率。ICC的表达与血β-hCG、CK、VEGF和激活素A存在相关性，可能是异位妊娠早期诊断的分子标记物。
